Plasmas esineb kolesterool lipoproteiinide koostises: k\u00fclomikronites, v\u00e4ga v\u00e4ikese tihedusega lipoproteiinides (<em>very low density lipoproteins,<\/em> VLDL), vahepealse tihedusega lipoproteiinides<em> (intermediate density lipoproteins,<\/em> <em>IDL<\/em>), v\u00e4ikese tihedusega lipoproteiinides (<em>low density lipoproteins,<\/em> LDL) ja suure tihedusega lipoproteiinides (<em>high density lipoproteins<\/em>, HDL). 60\u201370% kolesteroolist on LDL ja 25\u201335% HDL-i koostises. Kolesterool eritub organismist sapphapetena, v\u00e4ike osa ka sapi koostises muutumatul kujul. Kolesterool on oluline rakumembraanide komponent ning mitmete f\u00fcsioloogiliselt t\u00e4htsate steroidide (sapphapped, vitamiin D, steroidhormoonid) s\u00fcnteesi eel\u00fchend. Erinevatel lipoproteiinidel on erinev funktsioon ning seos kardiovaskulaarse riskiga. Kui LDL-i peamiseks funktsiooniks on kolesterooli transport kudedesse, siis HDL-il on selleks \u00fcleliigse kolesterooli tagasitransportimine maksa. LDL-i sidumine makrofaagide poolt arteri seinas on oluline etapp ateroskleroosi patogeneesis.<\/p>\n\n\n\n<p>Kolesterooli m\u00e4\u00e4ramisel m\u00f5\u00f5detakse erinevate lipoproteiinide koostises oleva kolesterooli koguhulk, HDL-kolesterooli ning LDL-kolesterooli on v\u00f5imalik m\u00e4\u00e4rata ka eraldi. Mitte HDL-kolesterool on arvutuslik parameeter, mis lisandub automaatselt kolesterooli ja HDL-kolesterooli olemasolul tellimuses.<\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong>Uuritav materjal, selle v\u00f5tmine, saatmine ja s\u00e4ilitamine<\/strong><\/p>\n\n\n\n<p>Paastumine enne prooviv\u00f5ttu ei ole reeglina vajalik. 3,0 mmol\/L<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>\u00dclaltoodud soovitatavate v\u00e4\u00e4rtuste aluseks on v\u00f5etud Eesti s\u00fcdame- ja veresoonkonna haiguste preventsiooni juhised (2006), Euroopa Kardioloogide Seltsi kardiovaskulaarsete haiguste preventsiooni juhis (2016) ning Euroopa Ateroskleroosi \u00dchingu (EAS) ja Euroopa Laborimeditsiini F\u00f6deratsiooni (EFLM) juhis (2019).<\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong>N\u00e4idustus ja kliiniline t\u00e4hendus<\/strong><\/p>\n\n\n\n<p>Aterosklerootilise kardiovaskulaarse haiguse riski hindamine kompleksis koos teiste riskifaktoritega (vt ka Trigl\u00fctseriidid), lipiidelangetava ravi j\u00e4lgimine.<\/p>\n\n\n\n<p>Soovituslikust k\u00f5rgem kolesterooli, LDL-kolesterooli ja\/v\u00f5i mitte HDL-kolesterooli ning v\u00e4iksem HDL-kolesterooli tase organismis suurendab s\u00fcdame-veresoonkonna haiguste riski.<\/p>\n\n\n\n<p>Lipiidelangetava ravi j\u00e4lgimine toimub LDL-kolesteroolil p\u00f5hinevate eesm\u00e4rkv\u00e4\u00e4rtuste alusel. Viimased&nbsp; s\u00f5ltuvad SCORE (<em>Systematic Coronary Risk Evaluation<\/em>) s\u00fcsteemil p\u00f5hineva&nbsp; riskihinnangu tulemusest.<\/p>\n\n\n\n<p>Kolesterooli tase organismis v\u00f5ib t\u00f5usta rasva- ja kolesteroolirikka dieedi, kuid samuti kolestaasi, nefrootilise s\u00fcndroomi, kroonilise neerupuudulikkuse, h\u00fcpot\u00fcreoosi, diabeedi ja \u00fclekaalulisuse korral. Raseduse ajal v\u00f5ib kolesterooli tase olla tavalisest k\u00f5rgem. Geneetilistest teguritest on olulisimad LDL-retseptori geenimutatsioonid (perekondlik h\u00fcperkolesteroleemia), mille korral v\u00f5ib juba varajases elueas kolesterooli tase olla t\u00f5usnud 7,5\u201312 mmol\/L-ni.<\/p>\n\n\n\n<p>Kolesterooli taseme v\u00e4henemine v\u00f5ib esineda alatoitluse, malabsorptsioonis\u00fcndroomi, taimerasvade tarbimise, pahaloomuliste kasvajate, maksatsirroosi ja h\u00fcpert\u00fcreoosi korral.<\/p>\n\n\n\n<p>Erinevate primaarsete d\u00fcslipideemiate puhul muutuvad HDL- ja LDL-kolesterooli v\u00e4\u00e4rtused erinevalt, nt perekondlikule h\u00fcperkolesteroleemiale on iseloomulik LDL-Chol suurenenud hulk, h\u00fcperalfalipoproteineemiale aga suurenenud HDL-Chol hulk. M\u00f5\u00f5dukalt kuni v\u00e4ljendunult t\u00f5usnud LDL-Chol tase v\u00f5ib tekkida sekundaarsena mitmete haiguste, nagu diabeedi, h\u00fcpot\u00fcreoosi, nefrootilise s\u00fcndroomi, kroonilise neerupuudulikkuse, samuti raseduse korral. V\u00e4henenud LDL-Chol v\u00e4\u00e4rtused v\u00f5ivad esineda raske \u00fcldhaigestumise ning m\u00f5ningate ravimite tarvitamise puhul, samuti m\u00f5ningate harvade p\u00e4rilike lipoproteiinide defitsiitide korral (abeetalipoproteineemia, h\u00fcpobeetalipoproteineemia). T\u00f5usnud HDL-Chol taset p\u00f5hjustavad samuti mitmed ravimid (\u00f6strogeenid, antiepileptikumid, bronhodilataatorid), rasedus ja krooniline alkoholi liigtarvitamine. Langenud HDL-Chol nivoo v\u00f5ib esineda \u00e4geda haigestumise, s.h m\u00fcokardiinfarkti ja insuldi, suurte traumade, hiljutiste operatsioonide, diabeedi, nefrootilise s\u00fcndroomi, kroonilise neerupuudulikkuse, h\u00fcpot\u00fcreoosi, maksahaiguste, aneemiate, m\u00fceloproliferatiivsete haiguste, Tangier\u2019i haiguse jm puhul.<\/p>\n\n\n\n<p>Mitte HDL-kolesterool annab aterosklerootilise kardiovaskulaarse haiguse riski hindamisel lisainformatsiooni olukordades kus veres esineb suurenenud VLDL- ja IDL-lipoproteiinide (ja neis sisalduva kolesterooli) hulk, kuid LDL-lipoproteiine on tavap\u00e4rases koguses.
