EPO reguleerib er\u00fctropoeesi vastavalt organismi hapnikuvajadusele. Hormooni vabanemist kontrollitakse neerude hapnikusensorite abil, mis on tundlikud vere hapniku osar\u00f5hule. Suurenenud perifeerse hapnikusisalduse korral v\u00e4heneb vere EPO kontsentratsioon ning vastupidi, hapnikuvaeguse ajal vabaneb seda suuremal m\u00e4\u00e4ral. On leitud, et EPO toimib ka v\u00e4ljaspool vereloomet \u2013 soodustab angiogeneesi, neurogeneesi, m\u00fcots\u00fc\u00fctide proliferatsiooni ja m\u00f5jutab kasvajate arengut. Terminaalse (l\u00f5ppstaadiumi) neerupuudulikkusega patsientidel on EPO tase \u00e4\u00e4rmiselt madal ning vereloomeks on vajalik asendusravi.<\/p>\n\n\n\n<p>EPO produktsiooni v\u00e4henemine infektsioonide, AIDSi, autoimmuun- ja p\u00f5letikuliste haiguste (reumatoidartriidi) ning pahaloomuliste kasvajate korral on p\u00f5hjustatud ts\u00fctokiinide (IL-1, TNF-a) p\u00e4rssivast toimest EPO s\u00fcnteesile.<\/p>\n\n\n\n<p>Madalaid EPO v\u00e4\u00e4rtusi v\u00f5ib esineda ka autonoomse neuropaatia korral ja 3\u20134 n\u00e4dalat peale luu\u00fcdi siirdamist.<\/p>\n\n\n\n<p><em>EPO kontsentratsiooni suurenemine<\/em><\/p>\n\n\n\n<p>EPO tase veres t\u00f5useb (p\u00f6\u00f6rdv\u00f5rdeliselt hemoglobiini v\u00e4henemisega) eriti k\u00f5rgele aplastilise-, hemol\u00fc\u00fctilise-, hematoloogilise kasvaja puhuse aneemia korral, m\u00f5\u00f5dukalt keemia- ja immunosupresiivravist indutseeritud aneemia puhul.<\/p>\n\n\n\n<p>Sekundaarsete pol\u00fcts\u00fcteemiate (madala hapnikusisaldusega \u00f5hus viibimine, krooniline obstruktiivne kopsuhaigus, kaasas\u00fcndinud s\u00fcdamerikked, omandatud s\u00fcdamehaigused, hemoglobinopaatiad) korral on EPO tase tavaliselt t\u00f5usnud v\u00f5i referentsvahemiku \u00fclemises osas.<\/p>\n\n\n\n<p>EPO s\u00fcntees v\u00f5ib s\u00f5ltumatult vere hapnikusisaldusest suureneda erinevate neeruhaiguste korral (ts\u00fcstid, h\u00fcdronefroos, neeruarteri stenoos, nefroblastoom (Wilmsi tuumor), neerurakk-kartsinoom, siirdatud neeruga patsiendid), tserebellaarse hemangioblastoomi, hepatotsellulaarse kartsinoomi ning emaka fibrom\u00fcoomi korral. EPO tase langeb tavaliselt peale kasvaja kirurgilist eemaldamist ning v\u00f5ib j\u00e4lle t\u00f5usta koos metastaaside tekkimisega. \u00dcksikutel juhtudel v\u00f5ib suurenenud EPO produktsioon esineda ilma er\u00fctrots\u00fctoosita, nendel juhtudel on tegemist inaktiivse EPO-ga (prekursoriga).<\/p>\n\n\n\n<p>Esilekutsutud pol\u00fcts\u00fcteemia p\u00f5hjuseks (sportlastel) v\u00f5ib olla autoloogne (oma) vere transfusioon, mille j\u00e4rgselt EPO sisaldus veres v\u00e4heneb, v\u00f5i eksogeense rekombinantse EPO (rHuEPO) kasutamine, mille korral EPO kontsentratsioon veres suureneb.
